Does my client qualifyfor your services?

To qualify for our Housing Stabilization Services (HSS), you must meet the following criteria:
✅ Have active Medical Assistance (MA)
✅ Be 18 years or older
✅ Experience housing instability (homeless, at risk of homelessness, or needing additional support to maintain housing)
✅ Have a documented disabling condition that impacts your ability to secure or maintain housing

How do I apply for assistance?

Call our front desk to check eligibility and sign up over the phone.

Click "Refer My Clients", complete the form, and email 360housingstability@gmail.com to confirm submission.

Expect a response within 24 hours regarding the next steps.

How long does it take to get approved?

The DHS approval process takes approximately 85 business days (around 3 months) due to a statewide backlog.

Once we receive approval, we will notify you or your case manager immediately.
